I voted D, because to me it depends on the number of characters and style of the show.
Like HIMYM is all narrated by Ted, and therefore he was the one introducing all the other characters and Robin wasn't even there at the very beginning, so you only had 3 other people.

A consecutive intro can be disturbing, because then the first show is only about character introduction (well of course they are) and no real story, which could interrupt the flow.
But the cons. introduction can be good, if it fits into the story.

I can never remember the names of characters, if there are more than 3-4, like Grey's Anatomy.
There it took me quite some time to remember them. Here they were introduced all at once, if I remember correctly, even though Meredith narrates parts of the show. But after the first episode you didn't really know much about the others, as there were too many of them.

In the end it should be the story, which tells you how to introduce the characters.

A bad example, if you've seen it: Twilight.
In the book it's all good, but there were too many characters in the movie.
You never really got to know Edward's siblings in the movie or Bella's friends. If you didn't read the book, you wouldn't really know anything about them and probably would have found it confusing. Who is who? Why are they even there?
Even though it was a slow unraveling.

So, my conclusion:
A: only if the narrator/main character keeps narrating the rest of the story. Or at least parts of it.
B: only if they are all equally important to the story, but this could be harder to grab people's attentions and disturb the flow.
C: If A nd B don't fit and I might prefer this one for books, because I want to get into the story right away and not be bothered with character intros I don't yet care about.
