All around her is chaos. It's clear that she's inside of the Uchiha complex of Madara's childhood and it's also clear that they are under attack. The Senju clearly must have located their complex and had decided to attack them in the middle of the night. Uchiha and Senju are fighting everywhere, both sides losing just as many people as the other.
Amongst the gore, if one pays close enough attention, a small figure can be seen running through the shadows. A young Madara, probably around four or five, moves silently through the shadows, leading a very young boy (who looks like a little Izuna who is just old enough to be walking) through the battle. He's far too young to fight on far with a fully grown Senju warrior, but their house had been attacked and they had to flee to one of the emergency shelters that the Uchiha kept.
Izuna clings to his brother, clearly scared about what's going on around him, while Madara keeps them moving ahead, careful to remain in the shadows so that they could remain undetected.
If Unohana chooses to follow them, she will see that they do indeed make it to the shelter, though there are several close calls that nearly get them caught.
It won't be long until the next memory begins.

She loses them as they disappear into the shelter and looks around again.
Blood, so much blood in Madara-san's life. She had an idea, of course, but to see so much, so early on ...
