letter of rec query

So, I know that it's pretty standard to send thank you cards to letter writers, but I have not done this when I've previously been recommended (bad me) and consequently have no idea what's supposed to go in them. Since some of you here have presumably been letter writers, and most of you letter receivers, how does this go?

In particular, one of my writers is super famous (and so, presumably, has flocks of people clamoring for her time) but has been totally supportive of me for, like, 4 years. She's revised her letter whenever I've needed to use it again, and all this after only taking one seminar with her. So, I feel like she's taken an above-and-beyond sort of interest in me, which feels relevant to how the card goes.

Help?
