Pain varies from woman to woman. There have been women here reporting no or very little pain from surgical, others from medical. Normally there is some pain. I'd say that most women I've seen report that surgical even with local anesthetic is quite painful, but only for a few minutes (it takes about 5-10 min). With medical, maybe 75% that I've seen have said it was quite painful (that was my experience) for some part of the miscarriage contractions (for me it was about 1 1/2 - 2 hours of the contractions that were very painful). With medical you may also vomit and have diarrhea during the miscarriage. (this is also common during labor, BTW - it's the same thing, a prostaglandin in your system causing contractions, only labor is worse).
The exception is that some (but not all) clinics offer general/twilight anesthesia for surgical abortion; that's where you're asleep, and it's painless, though there can be cramping or nausea after you wake up. Most charge slightly more for this sort of anesthesia because it's more dangerous and complicated. All the women who I've heard from who've done general anesthesia have been very glad they did.
I had a medical abortion (the pill kind, though I had methotrexate, a shot). The pain with that occurs after you insert the misoprostol pills into your vagina. They cause contractions, like labor (though not so bad), which last about 8 hours. They start mild and get stronger. About 2 hours in the middle are quite painful for most women.
Note about that: your friend shouldn't insert the pills right before bed or anything. She won't be able to sleep, which will add fatigue to the pain.
Medical abortion is better if your friend is afraid of the "surgery" (it's not cutting her open, but it does involve placing at least a straw through her cervix into her uterus). I don't like invasive procedures, which is why I wanted medical. Medical seems more natural to me. It's a miscarriage procedure as opposed to a surgical one.
Medical also takes place mainly at home (the miscarriage part). On the one hand, some women (like me) prefer to be at home in private, especially if there are supportive people around. On the other, there is pain and bleeding (your period starts) and no doctor around and that can be scary.
It's all personal preference, really... neither way is "The best" it's all "the best for you."
