18 November 2008 (1721 local): Initial “sacaojo” tag received to Tokyo office. Incidents in Arequipa, Peru involving two children under the age of fourteen. Eighteen tags received up to 27 February 2010.

25 February 2010 (2302 local): Employee 219 requests temporary overseas job. Specifies South American location.

27 February 2010 (0800 local): Employee 219 arrives in La Paz, Bolivia. Makes contact with Employee P51. Job designated PER5187.

2 March 2010 (0246 local): PER5187.9 Employee 219 reports possible human explanation to PER5187. Informs intended transfer to Arequipa, Peru.

8 March 2010 (0230 local): Employee 219 reports 76 different documented cases of “sacaojo” attacks between 18 November 2008 and 27 February 2010 (see PER5187.31-107). Short Detail: All victims under the age of fourteen. Two eyewitness accounts (unconfirmed). All victims appear to have both eyes removed, as well as extensive damage to surrounding area (to include zygomatic bone, sphenoid bone in ocular orbital socket, muscular damage). Each case also displayed additional damage to the body that prove fatal (see PER5187.151). No survivors.

12 March 2010 (1414 local): Employee 219 informs temporary transfer to Matarani, Peru.

12 March 2010 (2302 local): PER5187.170 Employee 219 reports contact with one of presumed eyewitnesses. Five year old male child. Matarani, Peru. Short Detail: victim no. 71 (sibling), 15-30 years of age, “foreigner”, short white hair, white skin, approx. 160-170cm height, approx. 54-68kg weight, “many eyes”. Employee 219 reports possible similarities to “Dodomeki” (see JAP988818417HOK8163).

13 March (0132 local): Employee 219 informs transfer to Arequipa, Peru.

17 March (0247 local): PER5187.178 Electronic file transfer to Employee 219.

25 March 2010 (0026 local): Employee 219 informs temporary transfer to Nazca, Peru.

25 March 2010 (0532 local): PER5187.198 Employee 219 reports two new victims two kilometers South of Nazca, Peru (confirmed). Male and female siblings. No survivors. Photographic evidence catalogued under PER5187.201 through PER5187.223. No eyewitnesses.

26 March 2010 (1643 local): Employee P51 reports mental instability for Employee 219 (unconfirmed): encounter with poisonous substance (unconfirmed) as a result of encounter with “sacaojo” (unconfirmed). Employee U51 requests forced medical examination for Employee 219.

26 March 2010 (1700 local): Contact with Employee 219. Employee 111 observes signs of tension, fatigue, possible stress. Employee 219 confirms encounter with possible poisonous substance (unconfirmed), denies encounter with “sacaojo” (unconfirmed), describes encounter with “cannibalizing chupacabra” (unconfirmed). Forced medical examination ordered. Employee 219 ordered return to Arequipa, Peru.

28 March 2010 (0120 local): Forced medical examination Employee 219 results: High stress. High tension. High fatigue. Malnutrition. Ramaria flavo-brunnescens poisoning. Underweight (possible anorexia nervosa), hallucinations, depression, ulcers, lesions on tongue (possible all related to R. flavo-brunnescens contact). Employee 111 confirms and supports medical order: 1 week bed rest, medication, low-stimulation environment. Expect complete disregard from Employee 219.

31 March 2010 (0230 local): Employee P51/U86/P22/B48 confirm Employee 219 is being a complete pain in the ass. All request to lift medical order as it does nothing: “it just gives 219 another way to make our lives miserable”. Order lifted.

3 April 2010 (1855 local): PER5187.224 Employee 219 reports new “sacaojo” incident (confirmed). Male, 7 years of age. Arequipa, Peru. Also reports possible unrest among locals concerning events and 219’s presence. 219 requests electronic file transfer (PER5187.226).

7 April 2010 (0200 local): Employee P51/U86/P22/B48 confirm no contact from 219 since 1400PET 3 April 2010.

8 April 2010 (0303 local): Employee P51/U86/P22/B48 confirm no contact from 219 since 1400PET 3 April 2010.

9 April 2010 (0200 local): Employee P51/U86/P22/B48 confirm no contact from 219 since 1400PET 3 April 2010. Peruvian and local authorities alerted. Additional employees from BA/BOL/URU ordered to assist.

10 April 2010 (0200 local): Employee P51/U86/P22/B48/BA1994/U21/B16 confirm no contact from 219 since 1400PET 3 April 2010.

11 April 2010 (0200 local): Employee P51/U86/P22/B48/BA1994/U21/B16 confirm no contact from 219 since 1400PET 3 April 2010.

11 April 2010 (2228 local): Employee 219 contact. Ordered full medical examination. Arequipa, Peru.

12 April 2010 (0004 local): Employee P51/U86/ B48/BA1994/B16 report concerns over Employee 219 mental stability. Employee P51/U86/P22/B48/BA1994/U21/B16 request 219 immediate transfer to Tokyo, Japan.

12 April 2010 (2353 local): Employee 219 contact with Tokyo. Adamant that he was present at Arequipa residence since 26 March 2010. Explanation as to why 7 employees from 3 different countries reported him missing for 8 days: “It is likely they were not paying attention”. Medical evaluation: Underweight, malnutrition, stress, exhaustion, paranoia, hallucinations. 219 to work under strict observation until further notice.

12 April 2010 (2353 local): PER5187.232 Employee 219 reports contact made with remaining eyewitness (unconfirmed): victim no. 65 (sibling), 15-30 years of age, short white hair, white skin, approx. 160-170cm height, approx. 54-68kg weight, “many eyes”, “the Devil”. Employee 219 reiterates local unrest over unresolved incidents (confirmed). 219 to work under strict observation until further notice.

14 April 2010 (1012 local): PER5187.247 Employee P51 reports Employee 219 contact with “sacaojo”. Subject has been disarmed and peacefully contained (unconfirmed). 219 advises no transport for subject. BA branch deployed to work with Peru branch.

14 April 2010 (1222 local): PER5187.248 Employee 219 ordered to Tokyo, Japan. Report due 2 weeks upon arrival in-country.

14 April 2010 (2030 local): PER5187.249 BA branch confirms subject disarmed and peacefully contained. Will transport to BA for further evaluation. Report due 21 April 2010.

21 April 2010 (0200 local): PER5187.250 Report received from BA branch (CLASSIFIED -see BA7332816).
