[The human mind does strange things, filling in memories where there are none just to keep itself together and make itself feel saner than it actually is.  So, while John Watson is use to hospital identification around, say, his wrist, the lack there of and presence of a collar suggests to him that this medical facility has switched tactics.  He doesn't quite approve.  Collars are choking hazards.  Still, having found what looks to be a mobile by his pillow and probably is a call button for the nursing staff, he decides to use this to question why he's here.  Sure, he was just in an explosion, but he feels fine.  He's more curious about Sherlock's status.]
