Wheee.. I made another tutorial cause spah said so :0

Crop your picture to an icon size.. like 96 x 96, I'll be using this picture of GERARD. Light pictures might work best

Sharpen the picturee [adjust - sharpness - sharpen]

Add some text. I used the font verdana, for the bigger text use size 4 and the smaller text size 1. And it's all in caps. Then merge visible

Duplicate the first layer and change the blend mode to screen and change the opacity to 50% or whatever you think looks best

Duplicate the first layer again and put it above all of the other layers. Then grayscale it [adjust - hue and saturation - colourize] Then change that layer's blend mode to soft light and keep the opacity at 100%, you can change it if you wantt

Now make a new layer. And flood fill it with #0A0836. Change it's blend mode to exclusion at 100% opacity

Use the gradient up there and paste it in your icon. Change the blend mode to burn and the opacity to 30% -ish

Take that gradient and put the blend mode on multiply with the opacity somewhere around 40%

Duplicate the gradient from step 8 and put the blend mode on overlay at 30%. Merge visible

Duplicate the layer and put the blend mode on screen then lower the opacity. Add stuff. And you're done
